Disability & Family Support
My Health Carers was established in 2000 and continues to provide quality care services to carers their families and disabled people living in our community. My Health Carers works in collaboration with programs such as Support and Choice ISP (Individualised Support Program), Family Choice, TAC & Work Cover by providing case management and direct care services.
My Health Carers prides it self on our personalised service and ability to match the right carer to clients individuality.
My Health Carers insure that our staff are professional and reliable.
All My Health Carers staff are required to have a minimum Cert 3 in Aged and Disability Care and receive ongoing professional training in areas such as OH&S, Personal Centred Care, Stoma and Bowel Care, PEG feeding and Hoist transfers.
All My Health Carers staff are required to undertake children and police check as a condition of their employment.
- These programmes include working with children & adults with disabilities or special needs and their families.
- Providing personal care showering
- Assisting with Home Care

- Providing respite for families and carers
- Taking client to programmes or appointments : day programmes and activities- support services, medical or hospital appointments.
- Assisting with Independant living skills. eg. :cooking, cleaning, shopping, paying household accounts, accessing internet & basic computer skills.
Arranging Social & Community activities: recreational e.g.: swimming, archery, gym., basketball etc...
Family Support Workers - assist a family who has a special needs child, with the daily living tasks. In some cases individual training is provided. i.e. PEG feed, Stoma Care, Hoist Transfers.
Community Access
Assisting individual to develop there own goals and aspirations. Improve peoples quality of life by assisting them to develop social support networks and access community activities and recreational programs.
- Arranging Social & Community activities: recreational e.g.: swimming, archery, gym., basketball etc...
- Socialization through community groups, activities and programmes
- Transportation: to groups activities and programmes
- Volunteer or Community Work and or paid employment Access camps, travel etc

Case management
Often people who need support at home also need the case management services.
My Health Carers provide qualified and experienced care managers to provide aged and disabilities with the following case management services.
My Health Carers provide Case Management services that are designed to provide the highest quality of support for people with disabilities.

Some of the services our Case Management Team can assist with include:
- Advocating for the rights of the Client.
- Coordinating rehabilitation programs.
- Referrals to Medical and Allied Health professionals.
- Negotiating with Legal and Advocate groups.
- Arranging Care Attendants.
- Developing lifestyle and social care programs.
- Occupational Therapy home visit assessments.
- Under the inhome care pg under respite and care support it needs to be broken up.
